Submit a Credit Application
Please download our credit application by clicking here. Note that you need to have the free software Adobe Acrobat Reader™ installed on your computer to be able to read and print the form. Please print the form, complete, and sign the credit application and the credit policy. Mail both documents to Flex Enterprises, Inc. at the address at the bottom of this page, Attention: Accounting Department. Please include your resale certificate and any additional documentation that you feel is necessary. We will process your credit application by conferring with your bank and credit references. Once the inquiries have been returned, we will examine the responses and determine an acceptable credit limit for your account. If you have any questions, please contact us.
